Patient/Client Consultation, Payment and Personal Information Storage
Age Well For Life Exercise Physiology Services needs to collect information about our patients for the primary purpose of providing a quality service to them. In order to provide the best possible exercise physiology services, we need to collect some personal information from our patients. If our patients do not provide this information; we may be unable to treat them. This information will also be used for:
The administrative purpose of running the practice; Billing either directly or through an insurer or compensation agency; Use within the practice if discussing or passing our patients case to another practitioner within the practice for their ongoing management; Disclosure of information to our patients doctors, other health professionals to facilitate communication and best possible care for our patients; and In the case of insurance or compensation claim it may be necessary to disclose and/or collect information that concerns our patients return to work to an insurer or employer.
We will take reasonable steps to protect the personal information we hold from misuse and loss and from unauthorized access, modification or disclosure.
While we take all due care in ensuring the privacy and integrity of the information our patients provide, The possibility exists that this information could be unlawfully observed by a third party while in transit over the internet or while stored on our practice database and online management system (halaxy.com). Upon initial consultation our patients will be asked to sign a consent form and given the option to agree to the fact we disclaim all liability to them to the greatest extent possible pursuant to law should this occur.
All Credit Card transactions are made via halaxy.com are processed securely using 256-bit SSL encrypted transmissions. We can take on the spot payment and store our patients credit card information on our halaxy.com management system for the convenience of prompt future transactions. Upon initial consultation our patients will be asked to sign a consent form and agree to the fact that they understand the reasons for the collection of their personal information and the ways in which the information may be used and disclosed and that they agree to that use and disclosure.
The possibility exists that private information could potentially be unlawfully observed by a third party while in transit over the Internet or while stored on the Age Well For Life Exercise Physiology practice management system (halaxy.com) and our patients are asked to read and sign a form (at initial consultation) on which they agree to the fact Age Well For Life Exercise Physiology Services disclaim all liability to them to the greatest extent possible pursuant to law should this occur.
