Funding For Clients Referred By Their GP Under A Chronic Disease Management Plan

Clients referred for clinically necessary treatment by their GP under a Chronic Disease Management plan ( Medicare Item # 10953) will receive a minimum rebate of $55.10* from Medicare from the total cost of their EP consultation. (Rebate should arrive in your bank account within 24 hrs). *Rebate amount may be higher depending on individual Medicare safety net threshold. A person who has a chronic or terminal medical condition can have a GP Management Plan (GPMP) service. A person with a chronic or terminal medical condition and complex care needs, requiring care from a multidisciplinary team, can have a GPMP and Team Care Arrangements (TCAs).

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. There is no list of eligible conditions.

However, a Chronic Disease Management Plan is designed for patients who require a structured approach to their care and to enable GPs to plan and coordinate the care of patients with complex conditions requiring ongoing care from a multidisciplinary team.

Funding For Clients Under Private Health

Exercise Physiology services are now recognised by almost all private health funds and substantial rebates are available (dependent on your level of cover). We recommend you contact your health fund to ensure you are eligible for Exercise Physiology services.

Funding For Clients Under Department Of Veteran Affairs

Department Of Veteran Affairs clients are bulk billed (no cost) for clinically necessary treatment with appropriate referral from GP.

Follow up visits allow your Exercise Physiologist to progress you on your exercise/rehabilitation program to ensure your body continues to adapt in a positive manner week after week. These sessions will include:

  • Progression of prescribed exercises
  • Review of goals and measurements 
  • Review of exercise technique and rehabilitation protocols  
  • Functional/clinical fitness retesting

Every individual client requires/prefers to commit to a unique volume and frequency of follow up visits.

Some clients prefer to see an EP multiple times per week for one on one personal training style workouts.

Some clients prefer to use the follow up visit at weekly, fortnightly or even monthly intervals. This method provides an opportunity for our EP to monitor our clients results and progress their program which they can then continue to undertake independently at home.

Age Well For Life EP can take on the spot payments via cash or card and also accepts bank deposits.


Cancellation Policy*

Age Well For Life EP requires a minimum of 48 hours notice if you wish to cancel your appointment. If you cancel or do not attend a scheduled Exercise Physiology appointment without providing 48 hrs notice, a fee will be charged.
