In-Home Disability Support For Improved Health and Well-being

When allocated funding under Support Category 3.12 Improved Health and Well-being you may utilise Exercise Physiology Service with Age Well For Life Exercise Physiology Services.

We provide FREE initial 30 minute meeting for NDIS Participants' in your home, this give us a chance to meet each other and set up the Service Agreement.

Our Exercise Physiologist is a registered provider with the National Disability Insurance Scheme (NDIS) on the Sunshine Coast/ Brisbane region and provides in home physical health and well-being services to people with a permanent disability.

Working in conjunction with a nominated NDIA planner or service provider coordinator, Age Well For Life EP will prescribe a comprehensive and specialised exercise regime, tailored to the specific needs of each individual.

If you are under the age of 65, reside on the Sunshine Coast or surrounding areas, have Australian residency and need support from a person or equipment to undertake your activities of daily living due to an impairment or condition that is likely to be permanent - then you may be eligible for assistance under the NDIS. 

Our services can be accessed through funding within a participant's plan under the Support/Capacity Building Category of Improved Health and Well-being and NOT “Improved Daily Living”. This is a common mistake that can cause your funds to be locked in another category. Plan amendments can be made to redirect funds into the Support/Capacity Building Category of Improved Health and Well-being if not already existing.

NOTE: Exercise Physiologists can improve functional capacity, in turn improving activities of daily living however AEPs are not approved under the ‘Improved Daily Living’ category. Currently, Exercise and Sport Science Australia (ESSA) are lobbying for changes to be made to the allocation of funding.

Patients who have the wrong allocated funding can request a plan review to their case manager to have a budget categorised under the “Health and Wellbeing” category for Exercise Physiology services.

Here is what you can expect from an exercise physiology consultation:

  • Medical health interview and assessment
  • Individual advice to a participant regarding exercise required due to the impact of their disability
  • Condition and goal specific functional assessment and physical activity testing
  • Short and long term goal setting and action plan
  • Development of a customised home based exercise/ rehab/ reablement program 
  • Ongoing graduated in-home exercise/ activity program and support


How many exercise physiology sessions will you require?

Clients that are independent with their exercise regime may only need a monthly consultation to assess progress and variety of programs. Other clients with more complex goals and physical or mental limitations affecting exercise may need weekly supervised sessions with an EP.

At Age Well For Life Exercise Physiology our aim is to enable NDIS patients to participate more in the community and to achieve their personal goals.

If you wish to become a participant in the NDIS, you must complete an Access Request Form, which is available by contacting the National Disability Insurance Agency (NDIA) on: 1800 800 110.

Once accepted with the NDIS individualized plans are created and aimed at the individual’s needs and aspirations. Participants will be able to choose their providers and how they access supports, as outlined in their individualized support plan.
